LEFC Forms
This page contains various forms used within LEFC.
Facility Use Form: This form must be completed in order to request use of any
part of the LEFC facilities or equipment. Completing this form does not guarantee that you will be authorized or that what you are requesting to use if available.
If you are requestion the ues of the facility please also download and read the Facility Use Policy before you complete and submit a request.
Event Promotion Form: Utilized by those in leadership at LEFC who are organizing or sponsoring an event that they would like promoted throughout the LEFC Family. This file contains the Event Promotion Process form, the Facility Use form and the Facility Use Policy so you should have everything you need to complete the request process.
These forms are fill-in forms for Adobe users. You should be able to open, complete, and print out the form all on your computer.
